Dementia Care Coach – Bilingual

There are nearly 18,400 people over the age of 65 in the Champlain LHIN region are living with dementia. Between 2012 and 2020, this number is expected to increase by 30 % to nearly 24,000. The Dementia Society of Ottawa and Renfrew County (DSORC) helps people in the Champlain LHIN region impacted by dementia by providing guidance and support, education, and social programs and peer engagement. We are hiring a Dementia Care Coach (DCC) to help us ensure that no one faces dementia alone.

The DCC will be responsible for providing support that is tailored to the individual needs of persons with dementia and their caregivers at any stage and at any age. Through our Dementia Link program, the DCC will provide outreach to clients, deliver support and education programs, and help build strong community and health care professional relationships.

Work Environment
This position works primarily at the Dementia Society’s central office in Ottawa. Hours are 8:30am until 4:00pm, with 30 minutes for lunch. Each DCC must be available to work one evening shift a week which is 12:30pm -8:00pm. Travel within the Champlain LHIN will be required.

Key Responsibilities
• Guided by Dementia Link program, identify interventions that help improve the quality of life experienced by people living with dementia and their caregiver’s through screening, assessment, and care planning.
• Support clients in navigating the healthcare system to access required services, support, resources, and learning opportunities
• Deliver education series, speaker’s bureau and other workshops to caregivers, persons with dementia and the general public.
• In partnership with volunteers, collaboratively lead monthly caregiver support groups.
• Collaborate with professional and community support services to support client goals, problem solve, overcome barriers and facilitate warm transitions between providers.
• Maintain a client-centered approach to support individuals, caregivers and families to get the right support, at the place and at the right time.

Qualifications and Skills
• Minimum of 5 years’ experience in health and/or social service sector. Regulated Health Care professional preferred. Minimum post-secondary education.
• Experience working directly with people living with dementia and their caregivers.
• Experience solving problems and engaging in quality improvement activities.
• Strong communication and organization skills.
• Demonstrates ability to take initiative and work independently.
• Counselling and psycho-therapeutic group facilitation an asset.
• Have a valid driver’s license and be able to drive across the Ottawa region.
• Bilingual in both French and English is required.
